ENROLLMENT PROCEDURES

NEW STUDENTS

1.1. Go to the Guidance Counselor for interview
1.2. Present admission requirements to the Assistant Dean.
1.3. See the Dean for final interview and get a Permit to Enrol.
1.4. Submit to the Registrar admission requirements and fill up the enrolment form.
1.5. Go to the Finance Office for assessment and payment
1.6. Present receipt of payment at the Registrar's Office for issuance of class cards

OLD STUDENTS

2.1. See the College Dean for approval of readmission.
2.2. Go to the Registrar's Office for subject loading,
2.3. Go to the Finance Office for payment of fees.
2.4. Present receipt of payment at the Registrar's Office for issuance of class cards

Section 3. College Registration Procedures

3.1 OLD STUDENT:

Step 1 Go to the clinic for physical check up & get Pre-registration Form.
Step 2 Confirm subjects to be taken from the Dean's Office.
Step 3 Claim Official Registration Form from the Registrar's Office.
Step 4 Proceed to the Finance Office for the Assessment and Payment
Step 5 Renew/validate School ID at the Student Affairs Office
Step 6 Claim Class Cards from the Registrar's Office.

3.2 RETURNEE STUDENT:

Step 1 Go to the Student Affairs Office for clearance.
Step 2 Go to the Guidance Office to update records.
Step 3 Go to the Clinic for physical check up & get Pre-Registration Form.
Step 4 Confirm subjects to be taken from the Dean's Office.
Step 5 Claim Official Registration Form from the Registrar's Office.
Step 6 Go to the Finance Office for the Assessment and Payment.
Step 7 Renew/validate School ID at the Student Affairs Office.
Step 8 Claim Class Cards from the Registrar's Office.

3.3 FRESHMAN AND TRANSFEREE

Step 1 Go to the Dean's Office for preliminary interview.
Step 2 Go to the Clinic for physical check up & get Pre-Registration Form.
Step 3 Pay testing fee & take test at the Guidance Center.
Step 4 Go to the Dean's Office to confirm subject to be taken.
Step 5 Get ID Number and Official Registration Form from the Registrar's Office.
Step 6 Go to the Finance Office for Assessment and Payment.
Step 7 Apply and ask validation of Identification Card from the Student Affairs Office.
Step 8 Claim Class Cards from the Registrar's Office.

3.4 Enrolment is final. Changing of subjects is not allowed. In the event that a student will change or drop a subject, a service fee will be charged.
